CM needed: where to apply to the fansite program? THX

Dear Blizz CMs,

I started an application to the FSP, and made everything it's written here:
http://us.battle.net/support/en/article/hosting-a-blizzard-fan-site

... but when I received the no-reply mail, it contained an improper link to the application form (error 404).

How can I apply my site to be an official Blizz Fansite? I mean where is the proper link to the webform?

There is currently no webform for submitting fan site applications, but you can send us an email instead :-)
